At Aloha Integrative Health, we try to do our best to provide the most up-to-date, transparent insurance information. While we do our best to verify your insurance benefits, we recommend that you also call your insurance(s) carrier to make sure every provider you see is covered under your plan. Depending on your insurance plan, it is possible for services to be covered in full or in part. Patient responsibilities may include deductibles, copays or coinsurance that are due at the time of each visit. Please contact the member service phone number on the back of your insurance card(s) to get an explanation of benefits for mental health services.

For those insurance plans that are Out of Network, we can provide you with an itemized receipt so that you can submit to your insurance company for reimbursement. Please note: this has to be determined before the time of your appointment.


We participate in some HMO plans which may require a written referral or authorization number from your Primary Care Physician.

For those who have trouble understanding insurance please contact your insurance company(s) and ask these questions which will help avoid the frustration of seeing a bill that you truly do not understand. It’s important to make sure you tell the insurance(s) that you are seeking Mental Health Services which may have a Carve Out Plan that handles this type of specialty.

Remember to document every call when speaking to your insurance provider including the date, rep’s name, and call reference number.


Questions to Ask Your Insurance(s)

  • Is Aloha Integrative Health In Network with my insurance plan? (Give tax id: 84-2965494)

  • Do I need a referral or an authorization for mental health services? If so, how do I obtain one? Make note of the authorization number, beginning date, end date, and the number of visits. 

  • What is my patient responsibility? Note both the copay and deductible price.

  • Is my Coordination of Benefits updated? Remember to always notify each insurance company when there is Other Health Insurance. Insurances determine which will be primary/secondary to avoid denials.

  • Do you have my updated mailing address for Explanation of Benefits to be mailed to me after my appt?


Information to Give When Scheduling

  • Provide name as it appears on insurance card.

  • Provide all medical insurances that are active at each time of service.

  • Student Health Plans are always secondary to a commercial plan. Please make sure that all medical plans covered by your employer or parents (give name and dob) are provided.

  • Social security number is essential to having a successful claim. Our system is encrypted and will not allow anyone to see this once inputted. For years insurances tried to move away from the social security but the billers still have many problems when researching your claim due to insurances not always up to the standards because old systems are still in place.
